RN MDS Coordinator - Marin Post Acute

Location: San Rafael, CA

Pay: $120,000-$140,000 annually (DOE)

Schedule: Full-Time

Marin Post Acute is seeking a skilled and detail-oriented RN MDS Coordinator to join our leadership team. If you are passionate about accuracy, regulatory compliance, and supporting outstanding resident care, we want to meet you!

What You'll Do

• Lead and manage the full MDS/RAI process in accordance with federal and state guidelines
• Accurately complete assessments, care plans, and documentation
• Collaborate with IDT members to ensure individualized, resident-centered care
• Monitor compliance, quality measures, and reimbursement accuracy
• Provide education and support to staff regarding MDS processes and updates
• Work closely with administration and clinical leadership on audits, reporting, and workflow improvement
  
  

What We're Looking For

• Registered Nurse (RN) with current California license
• MDS/RAI experience required (minimum 1 year preferred)
• Strong understanding of PDPM, care planning, and regulatory compliance
• Excellent attention to detail and organizational skills
• Strong communication and teamwork abilities
