What You'll Do
- Evaluate residents and develop individualized treatment plans
- Provide skilled physical therapy services to post-acute, rehabilitation, and long-term care residents
- Collaborate with physicians, nursing staff, therapy professionals, and families to ensure optimal outcomes
- Monitor progress and adjust treatment plans based on resident needs
- Educate residents and caregivers on mobility, safety, strengthening, and functional independence
- Maintain accurate and timely documentation in compliance with regulatory and reimbursement requirements
- Participate in interdisciplinary care planning and discharge planning activities
Required:
- Doctor of Physical Therapy (DPT) or equivalent degree from an accredited program
- Current California Physical Therapist license in good standing
- Strong clinical assessment and treatment skills
- Excellent communication and interpersonal abilities
- Skilled Nursing Facility, Post-Acute, Rehabilitation, or Long-Term Care experience
- Familiarity with Medicare documentation and reimbursement guidelines
- Experience treating orthopedic, neurological, cardiac, and medically complex patients
