LPN
Are you a compassionate and skilled nurse looking for a less stressful healthcare environment where you can build meaningful relationships with residents and their families? Margaret Pratt Community is seeking a phenomenal nurse to join our team
This role serves as a key support to our Health Services Director, helping provide exceptional care to residents in our nonprofit assisted living and memory care community.
Schedule: Full Time | Evenings 3-11 pm and/or Nights 11pm - 7am | Rotating Weekends

You'll play a critical role in resident care, medication management, and staff training. Your responsibilities will include, but are not limited to:
Resident Care & Assessment
- Conduct new and prospective resident assessments to ensure appropriate care planning.
- Update resident assessments annually or as needed when health conditions change.
- Develop and maintain individualized care plans for each resident.
- Provide staff training related to each resident's specific care needs.
- Assist with resident emergencies as needed.
Medication & Health Services Management
- Assist the medication administration program, ensuring accuracy and compliance.
- Coordinate with pharmacies to ensure a consistent and correct supply of medications.
- Administer medications to residents and manage medication pre-fills.
- Train and supervise staff responsible for medication administration, ensuring competency.
Staff & Operations Support
- Assist in creating and managing the nursing staff schedule.
- Provide ongoing training and oversight for caregivers related to resident care.
- Offer input on the evaluation of Health Services Assistants.
- Help review and update facility policies and procedures.
- Support the Health Services Director in managing the annual regulatory survey process.
Safety & Compliance
- Enforce infection control policies and procedures.
- Participate in emergency preparedness and safety programs within the community.
- Ensure compliance with HIPAA regulations regarding resident confidentiality.
What We're Looking For:
- LPN license in Vermont (Required)
- Experience in assisted living, memory care, or long-term care preferred
- Strong organizational and leadership skills
- Ability to work independently and as part of a team
- Passion for building relationships with residents and families
