We are seeking a Hospital Liaison who serves as the primary connection between hospitals, physicians, and our long-term care facilities. This role focuses on building strong referral relationships, coordinating patient transitions, and promoting our services within the healthcare community.
Key Responsibilities
- Develop and maintain relationships with hospital discharge planners, case managers, and physicians.
- Educate healthcare professionals about our long-term care programs and services.
- Coordinate admissions and ensure smooth transitions from hospital to facility.
- Conduct on-site hospital visits, presentations, and community outreach events.
- Track referral activity and maintain accurate records in CRM systems.
- Collaborate with internal teams to meet census and occupancy goals.
- Bachelor’s degree in healthcare, marketing, or related field (preferred).
- Minimum 2 years of experience in healthcare marketing, hospital liaison, or admissions (long-term care experience strongly preferred).
- Strong communication, relationship-building, and organizational skills.
- Knowledge of Medicare, Medicaid, and insurance processes.
- Ability to travel locally and work flexible hours as needed.
