Job Overview

Duties
- Administer medications and injections accurately following physician orders and established protocols, including IV infusions and medication administration.
- Perform comprehensive patient assessments, vital signs monitoring, and physical examinations to support diagnosis and treatment planning.
- Assist with medical procedures such as catheterization, suturing, tube feeding, and sterile processing techniques.
- Document patient care activities thoroughly using EMR (Electronic Medical Records) systems like Epic, Cerner, or Athenahealth to ensure compliance with HIPAA and other regulations.
- Provide specialized care for patients with complex conditions such as Alzheimer’s disease, dementia, traumatic injuries at Level I or Level II trauma centers, and infectious diseases.
- Support discharge planning and case management efforts to promote continuity of care in outpatient or post-acute settings.
- Collaborate with physicians, therapists, and other healthcare professionals in emergency medicine, ICU (Intensive Care Unit), pediatrics, geriatrics, hospice care, and behavioral health environments.
Requirements
- Valid state licensure as a Licensed Practical Nurse (LPN) with current certification to practice nursing in the respective jurisdiction.
- Proven experience working in diverse healthcare environments such as hospitals (including ICU and emergency room), nursing homes, long-term care facilities, or assisted living communities.
- Demonstrated proficiency with EMR/EHR systems such as Epic, Cerner, eClinicalWorks or Athenahealth for accurate medical documentation.
- Strong knowledge of medical terminology, anatomy and physiology principles, ICD coding (ICD-9/ICD-10), CPT coding, and diagnostic evaluation procedures.
- Experience with IV insertion/insertion techniques including IV infusion management and venipuncture procedures.
- Ability to perform patient assessments involving vital signs measurement and physiological monitoring; familiarity with ventilator management is a plus.
- Excellent communication skills for triage situations and working effectively with individuals with disabilities or developmental challenges.
- Knowledge of infection control standards and aseptic techniques essential for safe patient care delivery.
- Physical ability to stand for extended periods, lift patients safely using Hoyer lifts or similar equipment when necessary, and perform duties requiring manual dexterity.
