Duties And Responsibilities
- Assists patients in accessing insurance information and coverage through Medicaid and the Health Insurance Marketplace.
- Maintains certification as an Indiana Navigator
- Assist patients in accessing in-house affordability resources.
- Assist patients access health care financial assistance programs through Goshen Health
- Participates in community events to educate people about CAPS & Maple City’s navigator services
- Maintains a high level of accuracy, completeness, confidentiality, and integrity in all documentation.
- Documents patient contact using electronic medical record
- Documents patient interactions and services provided in the electronic medical record (EMR
- Utilizes telephone encounters, encounter notes, and other appropriate EMR documentation methods to accurately capture patient contacts and services.
- Performs other duties that support shared team goals and organizational priorities, as assigned.
- Coordinates and follows up with members of the care team as needed to support patient access and continuity of care.

Required Skills and Abilities
- Experience working with diverse groups of people
- Ability to take initiative, work independently, and collaborate with the team
- Ability to respectfully translate a variety of real-life situations into the Medicaid framework
- Fluently bilingual: English/Spanish; oral and written
- Effective interpersonal skills
- Associate degree strongly preferred, especially in social work or related field
- Certification as an Indiana Navigator
- Certified Application Counselor required (Training provided)
- Ability to perform prolonged periods of time in front of a computer
