The Floor Nurse is a member of a cross-disciplinary team that includes health care providers, medical assistants, nurses, behavioral health providers and other support staff. They support the mission and values of Maple City Health Care Center (MCHCC) by collaborating with team members across disciplines to facilitate appropriate quality care for patients while also empowering and encouraging self-motivation for a healthier lifestyle. Supervisory Responsibilities:
- None
- Collaborates with the cross-disciplinary care team to address the wholistic needs of patients
- Engages patients to address patient needs via phone/MyChart/in-person, utilizing Motivational Interviewing and Trauma Informed Care approaches
- Completes documentation in a timely and clear manner
- Refills patient medications
- Completes patient forms and letters in collaboration with provider
- Coordinates with outside entities for patient care
- Maintains skills competency by participating in scheduled trainings
- Other duties that support our shared work, as assigned

Required Skills and Abilities:
- Bilingual: Spanish/English, preferred
- Demonstrates effective interpersonal skills
- Nursing degree, BSN preferred
- State of Indiana licensure, Registered Nurse required
- Basic Life Support Certification required
- Trauma-Informed Care approach preferred, training supported
- Motivational Interviewing training preferred, training supported
- Ability to lift up to 20 pounds
- Ability to perform prolonged or extensive standing and walking
- Ability to perform considerable reaching, stooping, bending, kneeling, crouching
- Ability to perform prolonged periods of time in front of a computer
