Registered Nurse – Mental Health Reentry Program
Position Summary
Under the direction of the Program Director, the Registered Nurse provides comprehensive nursing care to justice-involved adults with mental health needs as they transition from detention into the community. This role supports stabilization, continuity of care, and connection to medical, psychiatric, and social services during post-release reentry.
Separately, we are also interviewing for an RN in a walk-in clinic working with adults over 18 with mental illnesses and co-occurring substance disorders.
Key Duties
- Deliver direct nursing care and post-release medical services
- Conduct health screenings, assessments, and testing
- Monitor client health status and medication compliance
- Coordinate primary care, specialty care, labs, and pharmacy services
- Identify psychiatric concerns and link clients to outpatient services
- Provide health education tailored to high-risk populations
- Participate in care coordination meetings and clinical consultations
- Maintain timely, accurate clinical documentation
- Meet productivity requirements for billable services
- Collaborate with multidisciplinary service teams
Qualifications
- Active California Registered Nurse (RN) license
- Minimum 3 years of RN experiencein psychiatric or related settings
- BSN may substitute for 1 year; MSN for 2 years
- Knowledge of mental health, psychiatric nursing, and justice-involved populations
- Strong communication, clinical judgment, and documentation skills
- Bilingual candidates preferred (testing required)
Requirements
- Current CPR certification
- TB clearance upon hire (maintained as required)
- Valid driver’s license; occasional client transport may be required
Pay
- 47- 52 an hour
