Job Title: WAGEES Case Manager

Reports to: Co-Executive Director of Programs and Operations

Position Status: Full-time, Non-Exempt, and Hourly; $25.00 an hour with a $1.00 an hour increase after successful attainment of Colorado Peer and Family Specialist (CPFS) credential.

Schedule: 36-40 hours per week; Monday-Friday 8:00 am - 4:30 pm (includes 30-minute lunch break)

Supervises: Program Participants

Location: The job cannot be performed remotely.

Position Summary: Manna is seeking a Case Manager to work in the CDOC – WAGEES community re-entry program. The Work and Gain Education & Employment Skills (WAGEES) Program provides support in the re-entry of individuals under the supervision of the Department of Corrections Division of Adult Parole and up to one year upon successful completion of parole to reduce recidivism and increase the chances of long-term success. Responsibilities include actively engaging participants in assessment, goal setting, resource navigation, and development of meaningful goals to increase education and employment skills while providing supportive services to participants. The WAGEES Case Manager will work with participants to overcome barriers of criminal justice system involvement, substance use disorder (SUD), and mental health and create a tangible path of increased stability and self-sufficiency to attain their goals. The WAGEES Case Manager will work with community members to grow current partnerships and build new collaborations to create a more comprehensive hub of social services/programming offered to support the program participants’ self-sufficiency, address basic needs, provide employment and housing security through a holistic approach to case management.

Duties and Responsibilities:

1. Determines and implements services and programs needed by participants that are consistent with the mission, vision, and strategic plan of Manna and the WAGEES program.

2. Supports individuals experiencing parole in community resource navigation. These include but are not limited to housing, employment, education, public assistance, mental health services, recovery support services and support groups.

3. Provide direct case management to enrolled program participants in the areas of education, employment, housing, goal setting, and support in accessing outside services.

4. Ensures all services provided are properly documented and accurately entered in required database (CaseMgr) and physical copies are stored securely in participant files.

5. Complete individual monthly progress reports and enter into CaseMgr as well as the CDOC database (eOMIS).

6. Conduct in-person case management meetings both in the office and in the community to work with program participants.

7. Complete intake assessments and collect required participant documentation to support assessment and intake in line with LCCL and DOC requirements.

8. Support program participants by acting as a resource for information about accessing Manna’s and other community programs and services.

9. Assist in co-facilitation of group services as needed.

10. Provide information and referrals to the Phoenix Life Skills class and provide support to YPR staff in organizing, recruiting, and placement.

11. Work closely with the Latino Coalition for Community Leadership and CDOC to facilitate the HSP transitional housing program, including screening of participants, coordinating with Spanish Trails for placement, and collecting and recording program fees.

12. Complete weekly check ins and room inspections for HSP transitional housing program participants.

13. Maintain consistent communication with Parole staff, CDOC Pre-Release case managers, and the Latino Coalition for Community Leadership for effective program delivery in line with required CDOC and LCCL policies and procedures.

14. Participate in Manna’s trainings, staff meetings, community meetings, and personal/professional development opportunities.

15. Complete all training and attain Colorado Peer and Family Specialist (CPFS) credential within six months of beginning position, all training costs and certification fees will be paid by WAGEES.

16. Attend and participate in meetings, webinars and training related to Manna, the WAGEES program and other relevant topics.

17. Learn and practice the support models guided by the Latino Coalition for Community Leadership, such as the INSIT model and Individualized Service Plans.

18. Coordinate resources and services with other Manna departments.

19. Assists in coordination of resources and services with other community agencies and non-profit organizations.

20. Contribute to maintaining an inclusive, safe, and supportive environment for staff, program participants, volunteers, and the community.

21. Maintain strict confidentiality per Manna and federal guidelines.

22. Set and adhere to strict boundaries and ethics when working with program participants.

23. Adhere to boundaries and strict ethics working with vulnerable populations, general public and partner agencies.

24. Maintain effective, consistent communication with the Co-Executive Director(s) to ensure program and organizational policies are being followed.

25. Encourage and assist participants in utilizing the WAGEES specific programs, including but not limited to the WAGEES E-bike Program, Tool Library, and Phoenix Life Skills Curriculum.

26. Assist in answering the door for visitors of the Administration Building.

27. Performs other duties as required or assigned by the Executive Director(s) and Resource Center Director.

Qualifications and Experience:

· High School Diploma or GED.

· Lived experience with substance use disorder or a mental health disorder and in active and sustained recovery or experience as a caretaker for an individual with a substance use or mental health disorder.

· Writing and oral skills necessary to communicate with staff, program participants, volunteers and community partners.

· Ability to utilize or learn office-related software and data entry programs.

· Working knowledge of confidentiality and mandated reporting requirements.

· Must maintain a current driver’s license.

· Pass a pre-employment drug screening and during employment if requested.

· Pass a criminal background check, a conviction will not automatically disqualify you from this position, and individuals with criminal justice involvement are encouraged to apply.
