Responsibilities
- Deliver one-on-one nursing care to patients in their homes, ensuring comfort and safety.
- Administer medications and treatments as prescribed by physicians.
- Monitor patient health and report any changes to the healthcare team.
- Assist with tube feeding and manage any necessary medical equipment.
- Provide health coaching and education to patients and their families regarding ongoing care needs.
- Support patients with developmental disabilities, fostering independence and quality of life.
- Collaborate with other healthcare professionals to develop and implement individualized care plans.
- Maintain accurate documentation of patient progress and care provided.
Experience
Candidates should possess the following skills and experience:
