Role Description The Primary Care Physician or Advanced Practice Registered Nurse will provide comprehensive, patient-centered primary care services to adults and adolescents, with a focus on integrating physical and behavioral health needs. Day-to-day responsibilities include conducting thorough evaluations, diagnosing and managing acute and chronic conditions, ordering and interpreting diagnostic tests, prescribing appropriate treatments, and maintaining accurate, timely documentation. The physician will collaborate closely with behavioral health providers and other members of the care team to coordinate treatment plans, support continuity of care, and facilitate referrals when appropriate. Additional responsibilities include participating in quality improvement initiatives, adhering to evidence-based guidelines, and contributing to a safe, inclusive clinical environment. This is a full-time, on-site role located in Somerset, KY.
Qualifications
- Licensed MD, DO, or APRN with board certification or eligibility in Family Medicine, Internal Medicine, or a related primary care specialty; valid Kentucky medical license or ability to obtain one.
- Demonstrated clinical expertise in diagnosing and managing common acute and chronic medical conditions in diverse populations, including experience with preventive care and health maintenance.
- Ability to integrate behavioral health considerations into primary care practice, including collaboration with therapists, counselors, nurse practitioners, and psychiatrists.
- Strong communication and interpersonal skills, with the ability to build trust, explain treatment options clearly, and work effectively in a multidisciplinary team setting.
- Commitment to serving rural and underserved communities, and to providing culturally sensitive, non-judgmental, and trauma-informed care.
- Proficiency with electronic health records (EHR), clinical documentation, and basic computer skills; comfort with using data to inform clinical decisions and quality improvement.
- High level of professionalism, reliability, and ethical standards, including adherence to regulatory requirements, patient safety protocols, and confidentiality guidelines.
- Residency training completed in a relevant primary care discipline; additional experience or training in behavioral health integration or community health is a plus.
