We are nationally recognized for our first-rate reputation and for delivering outstanding post-hospital rehabilitation and long-term nursing care. We are currently seeking a temp/PD Social Worker.
Position Requirements:
- Bachelor’s degree in Social Work or Human Services
- 1-year experience in a health care facility
- Experience in discharge planning
- Point Click Care preferred
Highlight of Responsibilities:
- Performs periodic assessments of all residents to determine status and needs.
- Handles resident concerns and assists residents in a respectful and non-judgmental manner, fostering independence and self-determination whenever possible.
- Participates in resident care conferences to assist in the implementation of care plans and assists with coordinating family council.
- Assists residents and families with adjustment that follow initial placement. Encourages resident participation in facility activities.
- Identifies and documents changes in behavior or personality. Advise the appropriate personnel.
- Meets with residents and/or families to complete all admissions paperwork. Verifies that all paperwork is in order.
