Role Description
- Travel to different facilities in a coordinated area to treat Geriatric patients
- Prepare and update patient records through our EHR system and publish test results to our online medical care portal
- Treat patients for internal medicine ailments and order blood work and other tests as needed
- Travel to patient home and different facilities such as Nursing Homes, Assisted Living to treat patients
- Establish rapport with patients and encourage them to talk with you candidly and frankly
- Provide advice on healthy habits and practices for patients based on their current health status
- Prescribe and refill medications as needed
- Provide referrals to specialists when required
- Assist practice physician when needed
- Perform physical examinations
- Create treatment plans
Qualifications
- Strong clinical skills, including patient assessment, health evaluation, and chronic disease management
- Experience in prescribing medications, developing care plans, and managing treatments
- Proficiency in healthcare documentation, patient records management, and care coordination
- Excellent communication skills for interacting with patients, families, and interdisciplinary healthcare teams
- Ability to provide compassionate, patient-centered care and adapt to diverse healthcare settings
- Current Nurse Practitioner certification and valid licensure to practice in Florida
- Minimum of 2 years of experience in geriatric or primary care practice preferred
- Master’s degree in Nursing (MSN) or related field required
- BLS/ACLS certification preferred
