- TASKS AND POLICIES:
- Coordinate and manage the community referral received.
- Serve as an integral member of the County’s Homeless Outreach Team and other community partners to assist in navigating individuals from homelessness.
- The essential functions include but are not limited to assessment/diagnostic skills, capacity to form relationships with members and maintain therapeutic alliance, assisting members in accomplishing their treatment plan goals and objectives, ability to provide a variety of treatment modalities: individual, group, family, case management, and crisis intervention.
- Coordinate and implement new programs, prepare case record documentation, monthly statistics, and other information as needed.
- Actively participate in staff and board committee meetings, develop a plan with each member, facilitate collaborative services needed and provide intervention counseling when necessary.
- Provide in-home and/or on the street consultation and crisis intervention with members, have an excellent grasp of theory and application as it relates particularly to homelessness issues and economic challenges.
- Must be able and willing to partner with law enforcement officers and walk and interact with homeless members of the community.
- Must be comfortable interacting with individuals in their home setting who need assistance navigating resources.
- Comfortable working with individuals in a mental health crisis and have the ability to remain calm.
- POLICY

- Operates and cares for equipment, tools, and vehicles according to prescribed standards and schedules.
- A bachelor’s degree from an accredited college or university in social work, psychology, sociology, human services, behavioral sciences, or related field.
- 1 year of work experience in case management or crisis intervention experience in a community or social service setting (non-clinical)
- Equivalent combinations of education and experience may be considered.
- 35 WPM typing required.
- Ability to lift up to 50 lbs. without assistance.
- Valid Florida driver license.
- Must pass MCSO 8-hour Driver Improvement Course.
