A Day in the Life of an Intake Coordinator
As an Intake Coordinator, you play a vital role in ensuring timely and accurate processing of Home Health and Hospice referrals. Your responsibilities include:
- Managing the face‑to‑face documentation process within required timeframes
- Completing all referral‑to‑start‑of‑care documentation
- Following up with physician offices to obtain necessary records, discharge instructions, and medication lists
- Processing referrals and ensuring all documentation is complete and compliant
- Maintaining confidentiality and adhering to state and federal guidelines
- Collaborating with the billing department to release held claims and resolve missing documentation
- Assisting with general office duties such as answering phones and distributing mail when needed
- Serving as a backup scheduler and entering referrals as required

Qualifications
- Associate degree in a related health or financial field
