Job Overview
We are seeking a detail-oriented and motivated Medical Record Auditor to join our dynamic team. In this vital role, you will be responsible for reviewing and ensuring the accuracy, completeness, and compliance of medical records. This role supports reimbursement optimization, audit readiness, and interdisciplinary documentation integrity across facilities.
Duties
- Perform routine audits of medical records to ensure completeness, accuracy, and compliance with federal and state regulations.
- Review documentation to support MDS assessments, including ADLs, physician orders, care plans, and clinical notes.
- Identify missing, incomplete, or inconsistent documentation and communicate findings to nursing staff and MDS coordinators.
- Maintain organized and up-to-date resident records (electronic and/or paper).
- Track audit results and prepare reports for the MDS team and administration.
- Assist with data entry related to MDS assessments and audit tracking logs.
- Ensure proper filing, scanning, and record retention in accordance with HIPAA and facility policies.
- Support MDS submission timelines by verifying that required documentation is present before submission.
- Collaborate with interdisciplinary team members to resolve documentation discrepancies
Requirements
- High school diploma or equivalent required; associate degree in healthcare administration or related field preferred.
- Previous clerical or administrative experience in a healthcare setting strongly preferred.
- Familiarity with MDS processes or long-term care documentation is a plus.
- Knowledge of HIPAA and medical record confidentiality requirements.
- Proficiency in Microsoft Office and electronic medical record (EMR) systems.
