Remote Temporary F/T IL Licensed (RN or LPN) Managed Care Case Managers Needed
Our client, a busy and growing Managed Care MSO. It is an advanced technology-driven health services platform that partners with health systems and clinical networks to bring scale and predictable performance to value-based care. Our client is seeking to hire 3 RNs and one LPN for long-term, temporary, remote full-time positions. Candidates must be IL RNs or LPNs with Managed Care case management experience. Someone coming out of a home health, hospice, worker’s comp, or hospital in-patient setting would not be appropriate for this need. Candidates should have full cycle CM experience in an outpatient setting, either on a payer or provider side.
Our client seeks RNs w/ recent Managed Care, experience dealing with chronic care health issues. These are temporary remote full-time positions working Monday-Friday 8 hours a day between the hours of (8:00am to 6:00pm).
Job Responsibilities
Up to 40 calls per day depending on engagement type/call outcome:
Transition of Care Calls:
· Verify eligibility
· Initial chart/EMR review
· Outreach within 48 hours
· Documentation of discussion/call status in the health plan platform
Condition Management Calls:
· Work the queues for asthma and diabetes
· Identify members that qualify for Tier II programs
· EMR review & Focused outreach
· Documentation of discussion/call status in the health plan platform
Job Requirements
· At least 2 years of recent Managed Care Case Management experience is required.
· 5 years of Nursing in either Med/Surg/ICU/ER would be beneficial.
· Must be an IL Licensed RN or LPN
· CCM is preferred, but not required.
· Must have full cycle case management experience in an outpatient setting.
· Must have experience dealing with chronic health conditions and determining long-term health care plans.
