Schedule

Part-Time: 3-5 hours per week with a weekly rotating on call responsibility.

Summary

The Nurse Practitioner (NP) provides on-site physical health services for individuals served in THRIVE’s NYS OASAS-licensed residential programs. The Nurse Practitioner will also be part of a rotating medical on-call coverage schedule which includes the three adult Part 820 Reintegration Community Residences and the youth Part 817 Residential Rehabilitation Services for Youth (RRSY) program. The NP plays a key role in integrating physical and behavioral health care, supporting recovery, and promoting wellness. This position works closely with nursing staff, counselors, case managers, and leadership to ensure high-quality, person-centered care.

Essential Duties And Responsibilities

Direct Clinical Care

  • Perform comprehensive physical health assessments for all new admissions.
  • Provide ongoing medical follow-up, chronic disease management, and acute care interventions within NP scope of practice.
  • Conduct medication management, including prescribing, renewing, adjusting, and monitoring medications.
  • Order and interpret laboratory tests, diagnostic studies, and specialty referrals.
  • Develop individualized treatment plans in collaboration with the interdisciplinary team.
  • Provide health education, preventive care, and wellness counseling.
  • Document all clinical encounters in accordance with OASAS regulations and agency policies.

Medical On-Call Responsibilities

  • Participate in a rotating on-call schedule to support, Three Part 820 Reintegration Community Residences and One Part 817 Residential Rehabilitation Services for Youth (RRSY) program
  • Provide telephone consultation to staff regarding urgent medical concerns, medication issues, and decisions about emergency care.
  • Respond promptly to calls and document all on-call interactions per agency protocol.

Collaboration & Compliance

  • Work closely with onsite nurse to ensure continuity of care and appropriate follow-up.
  • Participate in treatment team meetings, case reviews, and quality improvement initiatives.
  • Maintain the medical chart in accordance with agency policy and procedure
  • Ensure compliance with NYS OASAS regulations, HIPAA, 42 CFR Part 2, and agency clinical standards.
  • Support integration of physical and behavioral health services across programs.
  • Support staff training on medical topics, health protocols, and emergency procedures as needed.
  • Practice safety, environmental, and/or infection control methods.
  • Maintain standards of care as described in Principles of Practice, protocols and Scope of Practice as approved by the State of New York Board of Nursing and the American Medical Association.
  • Coordinate, as needed, with agency Medical Directors to review client cases and medical needs of the program.

Requirements

QUALIFICATIONS

EDUCATION And/or EXPERIENCE

Required: Current New York State license and certification as a Nurse Practitioner (FNP, ANP, or other appropriate specialty). Current DEA registration, Medicare and Medicaid Credentialed.

Preferred: Experience in substance use disorder treatment, residential care, community health, or primary care. Familiarity with NYS OASAS regulations. Strong clinical judgment, communication skills, and ability to work collaboratively in a multidisciplinary environment.

CERTIFICATES, LICENSES, REGISTRATIONS

Required: Must maintain valid driver’s license and acceptable driving record. Current New York State license and certification as a Nurse Practitioner (FNP, ANP, or other appropriate specialty). Current DEA registration, Medicare and Medicaid Credentialed.
