What We’re Looking For
As we continue to grow, we are seeking a dedicated and compassionate Registered Nurse (RN) to join our team. The ideal candidate will lead and inspire our caregivers, fostering a culture of empathy, respect, and professionalism. We are looking for an organized and forward-thinking RN who is passionate about providing top-tier care and who will play a key role in shaping the future of Magnolia Grace Senior Care.
As a Home Health Registered Nurse, you will:
- Provide admission, case management, and follow-up skilled nursing visits for home health clients.
- Report client care/condition/progress to client's physician and Clinical Manager on a continuous basis.
- Implement client care plan in conjunction with client and family to assist them in achieving optimal resolution of needs/problems.
- Coordinate/oversee/supervise the work of Home Health Aides, Certified Home Health Aides and Personal Care Workers and provides written personal care instructions/care plan that reflect current plan of care. Monitor the appropriate completion of documentation by home health aides/personal care workers as part of the supervisory/leadership responsibility.
- Discharge clients after consultation with the physician and Clinical Manager, preparing and completing needed clinical documentation.
- Prepare appropriate medical documentation on all clients, including any re-certifications, progress updates, and care plan changes.
- A nurse who genuinely cares about the well-being of our clients.
- Willing to participate in training (cost covered by us).
- Attend meetings via Zoom and in person.
- Collaborate closely with our Operations Manager to help keep things organized and running smoothly.
- Strong communication and follow-through.
Qualifications
- Diploma, Associate or Bachelor Degree in Nursing
- Current and unrestricted Compact Registered Nurse licensure
- Minimum of 5 years nursing experience
- Home Health experience a plus
- Current CPR/First Aid certification
- Strong organizational and communication skills
- Valid driver’s license, auto insurance and reliable transportation. Ability to travel to all business locations
