Overview:
We are seeking a dedicated Staff Pharmacist to join our team. The ideal candidate will possess a strong background in patient care, general disease management, and overall care pharmacy services.
Duties:
- Provide consultation and pharmaceutical care services to patients in accordance with outpatient policies and procedures
- Collaborate with healthcare providers to optimize medication therapy
- Conduct patient assessments and recommend appropriate drug therapy
- Participate in utilization management activities to ensure cost-effective medication use
- Demonstrate proficiency in critical care pharmacy practices
Requirements:
- Doctor of Pharmacy (Pharm.D.) degree from an accredited pharmacy program
- Active pharmacist license in the state of practice
- Strong knowledge of pharmacology, pharmacokinetics, and pharmacotherapy
- Ability to perform patient assessments and develop care plans
- Excellent communication and interpersonal skills for effective collaboration with healthcare teams
