Job Summary

We are seeking a dedicated and compassionate Hospice Social Worker to join our team in the Verde Valley area. The ideal candidate will possess a strong background in social work practice within hospice and palliative care settings, demonstrating expertise in patient assessment, end-of-life care, and community resource coordination. This role offers an opportunity to provide meaningful support to patients and families during challenging times, ensuring comprehensive psychosocial care aligned with clinical ethical guidelines and social work standards. The Hospice Social Worker will collaborate closely with interdisciplinary teams to deliver patient-centered services that promote dignity, comfort, and quality of life.

Responsibilities

  • Conduct thorough assessments of patients and their families to identify needs related to hospice care.
  • Develop individualized care plans tailored to each patient's circumstances.
  • Provide individual and group counseling sessions focusing on emotional support, grief management, coping strategies, and advance care planning.
  • Coordinate community resources such as assisted living facilities, in-home care services, etc.
  • Maintain accurate medical documentation in compliance with HIPAA regulations and social work practice standards while utilizing electronic medical record (EMR) systems effectively.
  • Educate patients and families about hospice & palliative medicine concepts, medical terminology, discharge planning, and available community services.
  • Participate in interdisciplinary team meetings to ensure holistic patient care and contribute clinical insights based on diagnostic evaluation and case management principles.

Requirements

  • Bachelor's degree in social work (BSW) or higher; licensure as a Licensed Clinical Social Worker (LCSW) or equivalent is preferred.
  • Proven experience working within hospice care or related healthcare environments such as hospitals, nursing homes, or outpatient clinics.
  • Demonstrated skills in dementia & Alzheimer's care, bereavement services, family counseling, and anticipatory grief management.
  • Familiarity with clinical methodologies related to hospice & palliative medicine is advantageous.
  • Excellent communication skills with the ability to perform intake assessments effectively while managing crisis situations compassionately.
  • Experience with EMR systems and inpatient/outpatient settings; assisted living experience is a plus.
  • Ability to work independently as well as collaboratively within multidisciplinary teams committed to delivering exceptional patient care across diverse populations including geriatrics and pediatrics.

Pay: $62,500.00 - $72,800.00 per year
