What are the responsibilities and job description for the Social Worker/Patient Advocate position at Madison Parish Hospital?
Madison Parish Hospital is currently accepting applications for one (1) Full-time Social Worker/Patient Advocate .
Bachelor's degree in social work, Counseling, Psychology, Human Services, or a related field required; master's degree in social work preferred.
Previous healthcare experience preferred.
Minimum of two years of experience in social work or case management within a rehabilitation, acute care, or skilled nursing setting. Must be able to coordinate community resources for patients and families served by the critical access hospital and collaborate closely with physicians, nurses, case management, and ancillary departments to support safe, timely, patient-centered care across transitional care settings.
Shift: 8:00 a.m.- 4:30 p.m.
