The Physical Therapist plays a central clinical role at Madison Medical serving as both an independent provider and a collaborative partner within our integrated care model. Schedule & HoursFull Time Monday- Friday Schedule with Rotating Saturday 8:30-1:30 shifts as needed
POSITION SUMMARYThis is a full-time Physical Therapist role focused on delivering high-quality outpatient care in a multidisciplinary setting.
You will evaluate, treat, and guide patients through structured rehabilitation plans. You will work closely with physicians and other providers to support coordinated, outcome-driven care.
KEY RESPONSIBILITIESPatient Evaluation & TreatmentPerform initial evaluations and reassessments
Develop individualized treatment plansDeliver manual therapy and therapeutic exercise programs
Provide functional movement training and mobility work
Educate patients on injury prevention and home programs
Adjust plans based on measurable progress
Documentation & ComplianceComplete accurate, timely EMR documentation
Use AI-assisted documentation tools and validate for accuracy
Maintain compliance with clinical and regulatory standards
Collaboration & CommunicationCoordinate care with physicians and providers
Communicate patient progress clearlyMaintain a professional, team-focused clinic environment
QUALIFICATIONSRequiredActive New Jersey Physical Therapist license in good standing
Current CPR/BLS certification4-5 years of outpatient, msk, sports, and or orthopedic physical therapy experience
Strong clinical reasoning and patient communication skills
PreferredExperience in a multidisciplinary medical setting
Familiarity with EMR systemsComfort working within structured workflows
