• Outcome 1: Deliver a "Patient Success Story" for >90% of your caseload through full Plan of Care completion and natural recovery.
• Outcome 2: Maintain a sustainable productivity standard of 60-65 visits per week.
• Outcome 3: Engage in 100% of our "Never-Skipped" weekly team meetings and twice-monthly 1-on-1s to drive your professional development.
