Position Summary

The Better Birth Outcomes Registered Nurse provides nursing assessments, intervention, case management, education, and care coordination services to eligible pregnant and postpartum clients. This position collaborates closely with WIC services, healthcare providers, community organizations, and families to support healthy pregnancies, healthy births, and improved maternal and infant outcomes.


Essential Duties and Responsibilities

  • Provide nursing assessments, referrals, follow-up, and case management services in collaboration with WIC and other community resources.
  • Conduct pregnancy, postpartum, and infant visits in accordance with program guidelines and nursing best practices.
  • Complete in-person visits, including home visits when requested by clients.
  • Assess eligibility for enrollment using income guidelines and the BBO-C Vulnerability Index.
  • Perform comprehensive physical assessments of pregnant and postpartum clients and infants, including vital signs, height, weight, and infant head circumference measurements.
  • Identify health concerns, provide appropriate interventions, and facilitate referrals for additional care and services.
  • Coordinate care with physicians, dentists, schools, community organizations, and other service providers as appropriate.
  • Collaborate with BBO team members regarding client needs and follow-up services.
  • Maintain current knowledge of community resources and evaluate referral outcomes.
  • Conduct outreach activities within the community, including local healthcare providers, FQHCs, pregnancy centers, prenatal clinics, and other community locations.
  • Complete required documentation and data entry in accordance with Illinois Department of Health and Human Services requirements.
  • Support the development of educational and outreach materials.
  • Assist with orientation, training, and job-shadowing experiences for new staff, interns, and students.
  • Maintain knowledge of all MCHD programs and provide appropriate referrals.
  • Serve as a mandated reporter of child and elder abuse.
  • Participate in emergency response activities as needed.
  • Adhere to confidentiality, HIPAA, and all MCHD policies and procedures.
  • Provide professional, compassionate customer service while representing the Macon County Health Department.


Qualifications

  • Registered Nurse (RN) with a current, unencumbered Illinois nursing license pursuant to the Illinois Nurse Practice Act.
  • Strong clinical assessment and case management skills.
  • Ability to build trusting relationships with pregnant individuals, families, healthcare providers, and community partners.
  • Excellent verbal, written, and interpersonal communication skills.
  • Ability to work effectively with diverse populations and demonstrate cultural awareness and sensitivity.
  • Strong organizational skills and attention to detail.
  • Ability to maintain accurate documentation and meet program reporting requirements.
  • Valid Illinois Driver's License with a clean driving record.
  • Reliable personal vehicle and current automobile insurance for work-related travel.
  • Ability to conduct home visits and community-based outreach activities.
  • Ability to pass a criminal background check, motor vehicle record check, and drug/alcohol screening.
  • Commitment to maintaining confidentiality and complying with HIPAA requirements.
  • Willingness to participate in emergency response activities and on-call situations as needed.
