GENERAL SUMMARY

Under direction, as member of an inter-disciplinary team for the Omnibus Budget Reconciliation Act (OBRA) department, conducts behavioral assessments and develops, implements and monitors mental health treatment plans for individuals served who are residents of nursing care facilities that have a Mental Illness (MI) and/or Developmental Disability (DD) diagnoses. Implements other program goals and activities as assigned.

ESSENTIAL FUNCTIONS AND RESPONSIBILITIES

Completes referrals and assessments and develops, implements and monitors treatment plans on assigned caseload.

Participates in case consultation to confirm diagnoses and to make the most suitable clinical recommendations.

Maintains appropriate contact with hospitals, physician offices and other service providers to coordinate consumer care and referrals.

Acts as a liaison for individuals served and/or their family members in obtaining authorization for appropriate programs and services provided, including the utilization and continuation of services.

Supports, consults, and monitors nursing home personnel and services as it applies to mental health treatment including consumer crisis intervention.

Maintains accurate and up-to-date records and clinical documentation following the Macomb County Community Mental Health policies and procedures.

Participates in agency and system staff meetings.

Serves on community mental health or community-based committees as directed.

Attends care planning conferences in nursing homes as well as maintains and updates clinical knowledge through readings, in-service trainings, and other activities as determined in consultation with the Agency’s annual plan and clinical supervisor or designee.

Performs administrative responsibilities of a Clinical Supervisor as needed.

May supervise the work of department interns.

Provides support and assistance to OBRA Coordinator as needed.

Operates an automobile to perform assigned job functions.

Performs related duties as assigned.

QUALIFICATIONS

Required Education and Experience

  • Master’s Degree from an accredited college or university in Social Work, Counseling, Psychology or Doctorate in Psychology or a directly related mental health field
  • Two (2) years of clinical experience working in a mental health setting

Preferred Education and Experience

  • Previous community mental health experience with children, adolescents, or adults
  • Knowledge of the Macomb County Provider Network

Required Licenses or Certifications

  • Licensed Master Social Worker (LMSW), Limited Licensed Master of Social Work (LLMSW), Licensed Professional Counselor (LPC), Limited License Professional Counselor (LLPC), Temporary Limited Licensed Psychologist (TLLP), or Licensed Psychologist (LP)
  • Possession of required state licensure appropriate to position and ability to meet privileging and credentialing guidelines.
  • Possession of a valid Michigan driver’s license and operable, insured automobile for authorized travel

Preferred Licenses or Certifications

  • Certification with the Michigan Certification Board for Addiction Professionals (MCBAP) or an approved development plan

  • Michigan Mental Health Code/401 criteria
  • Knowledge of OBRA nursing home reform legislation
  • Mental illness, serious emotional disturbance, and developmental disabilities
  • Techniques to de-escalate/interact in potentially dangerous situations
  • Applicable computer applications

GRANT OR EXTERNALLY FUNDED POSITION –This position will continue only if sufficient grant or external funds are provided.

Pay: $34.46 - $44.57 per hour

Expected hours: 37.5 per week
