Job Overview
We are seeking a highly skilled and compassionate Nurse Practitioner to join our healthcare team. The ideal candidate will possess extensive clinical expertise across various specialties, including acute care, internal medicine, geriatrics, pediatrics, emergency medicine, and behavioral health. This role involves providing comprehensive patient assessments, managing complex medical conditions, performing procedures such as suturing, IV infusion, catheterization, and wound care, and utilizing advanced EMR/EHR systems like Epic, Cerner, and eClinicalWorks. Experience with telehealth platforms and familiarity with ICD-9/10 coding, CPT coding, and utilization review are highly valued. The Nurse Practitioner will work in diverse settings including hospitals (Level I & II trauma centers), outpatient clinics, nursing homes, assisted living facilities, and post-acute care environments.
Responsibilities
- Conduct thorough physical examinations and patient assessments across various age groups including pediatrics, geriatrics, neonatal, and adult populations.
- Manage acute and chronic conditions such as pain management, infectious diseases, behavioral health issues, and post-operative care.
- Perform medical procedures including suturing, venipuncture, catheterization (including Foley and cardiac catheterization), tube feeding management, sterile processing techniques, and airway management such as ventilator use.
- Administer injections and medications accurately while adhering to HIPAA regulations and infection control protocols.
- Utilize EMR/EHR systems like Epic, Cerner, Athenahealth to document patient encounters meticulously.
- Coordinate diagnostic evaluations involving laboratory experience and medical imaging interpretation.
- Provide telehealth consultations utilizing telehealth experience to extend care remotely.
- Engage in patient education on health coaching topics such as nutrition, medication adherence, disease prevention (including Alzheimer's & dementia care), family planning, and discharge planning.
- Collaborate with multidisciplinary teams in hospital settings (including ICU/PICU/trauma centers) or outpatient clinics to ensure optimal patient outcomes.
- Participate in case management activities including utilization review and discharge planning for inpatient or outpatient populations.
- Maintain compliance with all healthcare regulations including NCQA standards and DME documentation requirements.
Requirements
- Valid Nurse Practitioner license with current state licensure; certification in Basic Life Support (BLS) required; ACLS preferred for ICU/emergency roles.
- Proven experience in diverse clinical settings such as hospital medicine (including ICU/ER), outpatient clinics, nursing homes (including hospice & palliative medicine), or home care environments.
- Demonstrated proficiency with EMR/EHR systems such as Epic, Cerner, eClinicalWorks; familiarity with ICD-9/10 coding and CPT coding is essential.
- Strong knowledge of physiology and anatomy; capable of performing diagnostic evaluations and physical examinations accurately.
- Experience administering injections (including IV infusion), performing sterile procedures like suturing or spinal taps; competency in venipuncture and catheter care required.
- Skilled in managing complex cases involving acute pain management, infectious disease care, occupational health issues, or senior/geriatric care including memory/dementia care.
- Ability to work independently within a multidisciplinary team; excellent communication skills for patient education and documentation.
- Commitment to infection control practices; experience working with vulnerable populations such as children or individuals with developmental disabilities is a plus.
