Job Overview
We are seeking a knowledgeable and compassionate Nutritionist/Dietitian to join our team. The ideal candidate will provide personalized nutrition care and support to patients, helping them manage health conditions and improve overall well-being through proper dietary planning.
Duties
- Assess patients’ nutritional needs and health conditions
- Develop individualized meal and nutrition plans
- Provide dietary counseling and education to patients and families
- Monitor and track patient progress
- Collaborate with healthcare providers to ensure coordinated care
- Support patients with conditions such as diabetes, hypertension, and obesity
- Maintain accurate documentation of care and progress
Requirements
- Registered Dietitian (RD) or Licensed Nutritionist/Dietitian (Connecticut preferred)
- Previous experience in clinical or home care settings (preferred)
- Strong communication and counseling skills
- Ability to work independently and manage time effectively
