Job Summary

Key Responsibilities
- Conduct in-home assessments to evaluate client needs, safety, and level of care required
- Develop and update individualized care plans based on client condition and needs
- Perform reassessments and supervisory visits as needed
- Monitor client status and report any changes
- Ensure services align with established care plans
- Maintain accurate and timely documentation
- Collaborate with caregivers and agency staff to ensure quality care
Qualifications
- Active Registered Nurse (RN) license in Virginia (required)
- Strong clinical assessment and care planning skills
- Ability to work independently and manage a flexible schedule
- Experience in home care, geriatrics, or long-term care preferred
- Excellent communication and organizational skills
- Reliable transportation for in-home visits
