Responsibilities Include:
- Evaluate the nutritional status of patients by reviewing pertinent information obtained from the electronic medical record, the hospital discharge papers, the interdisciplinary health care team, and family
- Apply knowledge of nutrition screening assessment to specific patient populations to define their nutritional needs
- Document pertinent findings in the medical record as per policy
- Provide quality nutrition counseling to patients, family members as indicated
- Implement and evaluate nutrition care plans for effectiveness; recommend changes as needed
- Use professional judgment to adapt as well as apply guidelines, protocols plus professional standards of care to the new changing needs of the patient
- Evaluate the need for enteral and parenteral nutrition support; providing recommendations as indicated
- Participate in interdisciplinary care plan meetings; communicating effectively with members of the health team
- Complete Minimum Data Set (MDS) documentation as requested by the facility
- Assist with discharge planning
- Menu planning as needed
- Knowledge of SIGMA Care preferred
Salary: $80,000.00 - $90,000.00 annually
