We are a high-volume, patient-centered Lymphedema Clinic dedicated to improving the lives of individuals affected by lymphedema and related conditions. Our team is passionate about providing compassionate care, and we are seeking a dynamic Patient Care Coordinator to join us in delivering exceptional service across our main and satellite locations.
Position Summary:
The Patient Care Coordinator plays a vital role in ensuring smooth clinic operations and a positive patient experience. This individual will be responsible for coordinating patient appointments, managing referrals, supporting clinical staff, and traveling to satellite clinics 1–2 times per week to maintain continuity of care.
Key Responsibilities:
- Schedule and confirm patient appointments.
- Coordinate care between providers, therapists, and external facilities.
- Maintain accurate patient records and documentation.
- Travel to satellite clinics to support patient intake, scheduling, and administrative needs.
- Communicate effectively with patients regarding treatment plans, insurance, and clinic procedures.
- Assist with front desk duties, including check-in/check-out and answering phones.
- Ensure compliance with HIPAA and clinic policies.
Qualifications:
- Prior experience in a healthcare setting, preferably in outpatient or specialty care.
- Strong organizational and multitasking skills.
- Excellent communication and interpersonal abilities.
- Proficiency in EMR systems and Microsoft Office.
- Ability and willingness to travel locally 1–2 times per week.
- Compassionate, patient-focused attitude.
Preferred:
- Experience in lymphedema, physical therapy, or rehabilitation clinics.
- Bilingual (Spanish/English) is a plus.
Job Type: Full-time
Pay: $16.00 - $18.00 per hour
