- Develop and maintain referral relationships with hospitals, physicians, case managers, and community organizations
- Review referrals and coordinate admissions processes, including assessments, documentation, and authorizations
- Conduct facility presentations and tours for referral sources and families
- Communicate resident needs to facility teams to ensure smooth transitions
- Promote a positive image of our facilities within the community
- Maintain knowledge of Medicare, Medicaid, and Managed Care processes
- Travel between facilities and throughout the Kansas City market to support referral development and admissions growth
- 2 years of admissions, business development, clinical liaison, or related healthcare experience
- Experience in skilled nursing, rehabilitation, post-acute care, or hospital settings preferred
- Strong communication, organizational, and relationship-building skills
- Professional customer service and phone skills
- Knowledge of Managed Care processes preferred
- LPN license preferred but not required
- Reliable transportation and willingness to travel throughout the Kansas City area
