Role Description
This is a full-time, on-site Case Manager role based in San Bernardino, CA. The Case Manager will be responsible for providing direct support to individuals and families, including assessing client needs, developing and implementing service plans, and connecting clients with relevant resources. Additional responsibilities include maintaining accurate case records, conducting follow-ups, advocating for clients with service providers, and participating in community outreach. The Case Manager will work collaboratively with a team to ensure clients receive quality care and services that promote self-sufficiency and dignity.
Qualifications
- Case management and client support experience, including service plan development and providing direct assistance to clients
- Strong interpersonal and communication skills, with the ability to build trust and foster professional relationships
- Organizational and administrative skills, including maintaining accurate documentation and managing case files
- Problem-solving and critical thinking skills to effectively address client needs and challenges
- Experience with community outreach, knowledge of local resources, and the ability to network with service providers
- Proficiency with technology and software for case management and communication
- Understanding of and commitment to social justice, diversity, equity, and inclusion
- Bachelor's degree in social work, human services, or a related field is preferred
- Experience in a nonprofit or human services setting is a plus
- Fluency in multiple languages is an advantage but not required
