Lutheran Social Services of Wisconsin & Upper Michigan is seeking a dedicated Care Coordinator to join our CLTS Waiver Program team in Walworth County!
The Childrenโs Long-Term Support (CLTS) Waiver Program is a federally funded Medicaid initiative that helps children with developmental, physical, or severe emotional disabilities receive services that allow them to live safely in their homes and communities. As a Care Coordinator, youโll be the primary point of contact for families, helping them access and navigate essential services with compassion and care.
๐ง What Youโll Do
- ๐ง Assess childrenโs functional abilities using approved tools
- ๐ Develop and implement individualized service plans with families and providers
- ๐ค Facilitate team meetings and coordinate services based on family-centered goals
- ๐ Maintain accurate documentation and meet all regulatory timelines
- ๐งญ Collaborate with internal teams and external agencies to support families
- ๐งโโ๏ธ Testify in legal proceedings when required
- ๐ฌ Communicate clearly with families, providers, and team members
- ๐งโ๐ Participate in staff development, training, and supervision

๐ Qualifications
- ๐ Bachelorโs degree in Social Work or a related human services field (e.g., Psychology, Special Education, Counseling, etc.)
- ๐ง Minimum 1 year of experience working with children and/or youth with disabilities
- ๐ Bilingual fluency in Spanish is preferred but not required
- ๐ป Proficient in computer systems and electronic health records
- ๐ค Strong interpersonal and organizational skills
- ๐ Valid driverโs license and reliable transportation (MVR check required)
๐ Work Environment
- Community-based with daily travel required
- Moderate noise level; occasional exposure to outdoor conditions
- Flexibility to meet family needs, including crisis response
