What are the responsibilities and job description for the Case Manager position at LUTHERAN SOCIAL SERVICES OF THE NAT?
The Case Manager will provide extensive case management to newly arriving refugees being received by LSSNCA's team. The Case Manager will ensure eligible clients are enrolled in appropriate programs and oversee the requirements of those programs. The objective of the Case Manager is to ease the transition, empower the refugee, and put the client on the path to self-sufficiency
Essential Functions
Ensure all core services mandated through the Department of State Cooperative Agreement are provided within allocated timeframes.
Complete suitability determinations for minor cases.
Provide extended case management and support services to clients enrolled in Public Private Partnership services.
Contact area service providers through mailings, phone inquiries, and person-to-person meetings to provide them program information and to inquire about available services. Make referrals as appropriate.
Assist eligible clients in the completion of Affidavits of Relationships and refugee interests.
As appropriate, request and client stipend checks.
Assist in the collection of program service data for reports,
Complete all forms as required
Maintain accurate record of all services provided.
Produce and submit reports in a timely manner.
Coordinate services with other area agencies.
Work as part of the program team and participate in staff meetings and in-service trainings.
Complete other tasks and projects as assigned.
Education: bachelor’s degree (four-year college or technical school) or Work Equivalent,
Field of Study: social work; human services
Experience: 1 plus years of experience in refugee resettlement
Other Requirements: Fluency in one or more of the following languages - Dari, Pashto, Spanish, Arabic, or Tigrinya; Ability to work in a diverse environment
