Monday through Thursday 6:00am-2:00pm
Sunday 8:00am-4:00pm

- Provide case management for the clients.
- Responsible for encouraging, monitoring, and maintaining a therapeutic environment.
- Responsible for outreach to other agencies to increase referrals and engagement from the community.
- Assess the client’s ability to meet admission criteria and placement.
- Compete intake packets for admission into program.
- Schedule intakes and walk-throughs.
- Conduct weekly discussions with clients regarding program fees.
- Demonstrate professional, positive behavior and carry out responsibilities with integrity, treating clients, families, other LSSI workforce members, and collaborative organizations and/or individual in a dignified, respectful, honest, and fair manner.
- High school diploma or equivalent preferred.
- Background check clearance required.
- Annual TB test required.
- Ability to effectively utilize Electronic Health Records (EHR) as it an essential tool to complete clinical documentation and other job functions.
- Ability to utilize NOWPOW to conduct case management for the clients.
- Customer Service experience preferred.
- Demonstrated ability to communicate in a clear, comprehensible manner, both verbally and in writing.
- Excellent organization, presentation, and pc/computer skills, including experience using Microsoft Office (Outlook, Teams, Work)
- Valid driver’s license, in good standing for the state of residency preferred.
- Access to reliable transportation preferred.
- Valid IL statutory minimum liability insurance coverage, bodily injury, and property damage preferred.
