Clinic Manager, LPN - $6,000 Sign-On Bonus (to be paid throughout the first year of employment).

LIFE Beaver County, PA - Full-time, Daylight 8:30 a.m. to 5:00 p.m.

Our services include LIFE (Living Independence for the Elderly); skilled nursing, home health; personal care; adoption & foster care; assisted living; adult day services; memory care; hospice; palliative care; residential living; affordable housing; home safety monitoring; Meals on Wheels; rehabilitation services and private duty.

LIFE is a program that serves county residents living in Armstrong, Butler, Beaver and Lawrence Counties. It is an all-inclusive comprehensive program that helps individuals maintain their independence in their home and community through supportive services. The LIFE programs offer an array of person-centered services from medical care to adult day supports with the primary goal of allowing individuals to remain in their own home.

Under the supervision of the Regional Director, manages the flow of the clinic, leading all staff in provision of medical and nursing care, ensuring high quality standards and regulatory compliance.  Coordinates services with primary care team and acts as a primary liaison for the clinic and nursing staff.  Triages participants as needed and is the gatekeeper for daily clinic scheduling.  Provides basic care to participants; implements physician, physician assistant and nurse practitioner orders.  Provides training and education to participant, caregiver family and staff; and communicates participant’s condition to caregivers, primary care physician, and the interdisciplinary team.  Has direct oversight of all responsibilities for the staff in the clinic, (excluding providers) including clinical educations, training/observations, reporting requirements, and competency evaluation.

Qualifications

Clinic Manager, LPN qualifications include:

• A Licensed Practical Nurse, must have graduated from an accredited school of nursing and possesses a current license, in good standing, from the Commonwealth of Pennsylvania.
• 2 to 3 years of nursing experience, including 1 year supervisory experience preferred, with 1 year experience working with a frail or elderly population preferred.
• Working knowledge of computers, Microsoft Word and Excel required.
