Summary: The Enrollment Specialist obtains initial information relating to potential enrollment, coordinates the intake process, and provides information and referrals to community agencies; Oversees the enrollment process of prospective participants through the level of care process as determined by the State of Missouri. Essential Duties and Responsibilities: Interacts with referrals sources, community organizations, caregivers, families, and prospective participants, giving presentations, leading tours of and sharing information about New Horizons PACE. Determines enrollment eligibility through screening, phone intake and education. Gathers critical information related to medical and social needs for prospective participants Serves as the first point of contact and liaison between the prospective participant and the interdisciplinary team (IDT). Assists the prospective participant, caregiver, and family with gathering and obtaining necessary documentation and information needed for enrollment. Explains the enrollment agreement to the prospective participant, caregiver, and family in a way and language that they can understand. Qualifications, Knowledge, Skills, and Abilities: To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. The requirements listed below are representative of the knowledge, skill and/or ability required. -Bachelor’s degree in health care, social science, marketing, or business or equivalent -One (1) year experience working with the elderly -Current American Heart Association Basic Life Support (BLS) with Automated External Defibrillator (AED) certificate or the ability to obtain within two weeks of hire, before having direct participant contact -Medical clearance for communicable diseases and up-to-date immunizations, before having direct participant contact -Excellent oral and written communication skills and perform public speaking engagements and community presentation -Ability to interact professionally and respectfully with other adults, caregivers, and families of all cultural and socioeconomic backgrounds, and those with cognitive decline or mental health issues -Ability to work effectively within an interdisciplinary team model, interfacing and collaborating with a wide range of clinical and social services disciplines who work together to manage the PACE participants' care -Ability to maintain accurate records and to prepare clear and concise reports, correspondences, and other written materials -Effective organizational skills with attention to detail and time management skills -Effective analytical and problem-solving skills -Proficient with Microsoft Office Suite or related software Physical Activities and Demands; Working Environment: -Prolonged periods sitting, standing, and walking outside the field -Ability to squat, crouch, bend, kneel, and crawl occasionally -Ability to push, pull, reach above shoulders occasionally -Ability to frequently lift/carry up to 10 pounds, occasionally lift/carry up to 25 pounds using appropriate body mechanics with reasonable accommodation if needed -Frequently use hands for simple and firm grasping and fine manipulation -Specific vision abilities required by the job include close vision, distance vision, color vision, peripheral vision, depth perception, and the ability to adjust focus -Exposure to bodily fluids, bloodborne pathogens, and/or other infectious illnesses common in a healthcare environment -Exposure to hazardous chemicals such as disinfectants, cleansers, soaps, etc. -Exposure to fluctuating indoor temperatures -Noise level in the work environment is usually moderate to loud -Subject to unpleasant odors -Potential exposure to verbal aggression by participant -Frequent local travel 75% of the time This job description in no way states or implies that these are the only duties to be performed by the employee occupying this position. Additional Information: Promote awareness within STL City and County about EverTrue's PACE program through facilitating outreach presentations. Answer inquiry calls and host tours at the PACE center.
