What are the responsibilities and job description for the SOCIAL WORKER position at Lutheran Living Services?

Lutheran Home is currently seeking a Social Worker for a full-time opportunity. The Social Worker assures that our resident's psychosocial needs are identified and met. Interested candidates should be organized, compassionate and dedicated to our mission and "4 C's."

Hours for the position are Monday - Friday 8:00 AM - 4:30 PM.

Essential Functions of the Job Include:

Assess psychosocial needs of residents
Develop and monitor care plans
Identify and document changes in resident condition
Facilitate admission/discharge process
Communicate effectively with residents/clients, visitors and interdisciplinary team

Requirements for the Job Include:

Bachelor's degree in Social Work
1 year experience preferred, however new social workers are encouraged to apply
Prior experience working with the senior population preferred
