Position Summary

  • Assertive Community Treatment (ACT) is an evidence-based practice that improves outcomes for adults with severe and persistent mental illness (SPMI) who are most at-risk of psychiatric crisis, hospitalization, and involvement with the criminal justice system. The ACT Mental Health Therapist provides therapeutic services and clinical support as part of the Assertive Community Treatment (ACT) team, focusing on the recovery and well-being of individuals with serious mental illnesses. This individual works closely with other ACT team members to ensure comprehensive, person-centered care that supports clients’ recovery and well-being.

Job Duties

  • Deliver evidence-based therapy and conduct comprehensive mental health assessments and diagnostic evaluations.
  • Develop individualized treatment plans in collaboration with clients and the ACT team.
  • Provide therapeutic interventions, including individual, group, and family therapy, utilizing evidence-based practices.
  • Provide on-call support and intervention on a rotating schedule, including evening, weekend, and holiday hours as scheduled.
  • Respond to psychiatric crises, providing de-escalation, intervention, and stabilization services as needed.
  • Partner with emergency services and other providers to ensure ongoing care during crises.
  • Assist clients in developing crisis response strategies and safety plans.
  • Transport clients to medical appointments or community resources as needed.
  • Work closely with the ACT team, including psychiatrists, nurses, and peer support specialists, to deliver integrated care.
  • Sort and distribute medications with the team and deliver to individuals. Follow medication procedures as set by Nurses and/or Prescribers (such as observation procedures). Document appropriately within electronic health record and maintain medication logs.
  • Participate in all team meetings to review client progress, address challenges, and adjust care plans as necessary.
  • Provide clinical expertise and consultation to the team regarding therapeutic approaches and interventions.
  • Deliver therapy and support in various community settings, including clients’ homes, workplaces, and other locations.
  • Assist clients in accessing community resources and building natural support systems.
  • Promote recovery-oriented, trauma-informed care that fosters independence and wellness.
  • Complete all required documentation in a timely and accurate manner, including but not limited to treatment plans, progress notes, and discharge summaries.
  • Participate in quality improvement initiatives and ensure fidelity to ACT model standards.
  • Other related job duties as needed.

Required Skills/Abilities

  • Expertise in mental health assessment, diagnosis, and therapeutic intervention.
  • Strong communication and interpersonal skills to engage clients, families, and other staff.
  • Ability to manage crises and provide effective, timely interventions.
  • Proficiency in electronic health record systems and Microsoft Office or related software.
  • Commitment to trauma-informed, culturally competent care.
  • Ability to guard confidentiality, including client information and proprietary business details.
  • Ability to ensure services provided are billable and coded properly.
  • Ability to deliver services to clients using an interpreter or via telehealth platforms.
  • Participate in peer reviews, routine clinical supervision, and Quality Improvement activities.
  • Regular and predictable attendance, and promptness for work.

Education And Experience

  • Master’s degree in social work, counseling, psychology, or a related field from an accredited university required.
  • Fluent in English and another language preferred (desired languages align with client populations, including but not limited to Spanish, Arabic, etc.).
  • Valid Driver’s license, proof of liability auto insurance, and ability to transport clients safely in personal vehicle on a regular basis.
  • Demonstrated skills and competencies to work with individuals with a variety of behavioral health, including but not limited to persons of diverse cultural backgrounds.
  • Obtain and maintain appropriate licensure within a reasonable timeframe (see licensing requirements below)

Position

License Required

Therapist I

One provisional license - Mental Health Professional (PLMHP) or Provisional Certified Master Social Worker (PCMSW) required; will consider PLADC.

Therapist II

Two provisional licenses - Mental Health Professional (PLMHP), Provisional Certified Master Social Worker (PCMSW), Provisional Alcohol and Drug Counselor (PLADC) required.

OR fully licensed with one of the following: LMHP, LADC, or LCSW

Therapist III

Fully licensed with a Licensed Mental Health Professional (LMHP), Certified Master in Social Work (CMSW) / Licensed Clinical Social Worker (LCSW), and/or Licensed Alcohol & Drug Counselor (LADC) required, plus one additional license of the following: Provisional Certified Master Social Worker (PCMSW), Provisional Alcohol and Drug Counselor (PLADC), Provisional Certified Master Social Worker (PCMSW), etc.

Therapist IV

Fully licensed with one independent license preferred (Licensed Independent Mental Health Professional (LIMHP) and/or Licensed Independent Clinical Social Worker (LICSW).

Fully licensed with 5 years of experience and two full licenses required (Licensed Mental Health Professional (LMHP), Licensed Clinical Social Worker (LCSW), and/or Licensed Alcohol & Drug Counselor (LADC))

Physical Requirements

  • Prolonged periods of standing, walking, driving, and sitting at a desk and using a computer.
  • In-office work and field environment with travel to other worksites and/or community sites.
  • Company-issued laptop and cell phone.
  • Provide on-call support and intervention as needed, including evening, weekend, and holiday hours.
  • Valid driver’s license, proof of liability auto insurance, and ability to drive a personal vehicle on a daily basis to transport clients safely within the community.
