We are seeking a dedicated Hospice Social Worker to join our team in a full-time, on-site role based in Wauwatosa, WI. The Hospice Social Worker will provide psychosocial assessments, emotional support, and counseling to patients and their families, particularly focused on end-of-life care. Key responsibilities include coordinating discharge planning, offering grief counseling, and collaborating with interdisciplinary teams to ensure patients receive compassionate and comprehensive care throughout their hospice journey.
- Expertise in Medical Social Work and knowledge of Social Work practices
- Experience in providing End-of-Life Care and Grief Counseling
- Ability to coordinate Discharge Planning effectively
- Strong interpersonal, communication, and organizational skills
- Master's degree in Social Work (MSW) or related field required
- Valid state licensure for social work
- Previous hospice or healthcare experience highly desirable
