Position Overview:
We are currently seeking a licensed and experienced Registered Nurse (RN) Case Manager to join our caring and dedicated team. This role is crucial to ensuring patients and their families receive high-quality, compassionate care tailored to their unique needs. We are currently seeking a licensed and experienced Registered Nurse (RN) Case Manager to join our caring and dedicated team. This role is crucial to ensuring patients and their families receive high-quality, compassionate care tailored to their unique needs. Responsibilities:
- Conduct thorough assessments of patients' physical, emotional, and psychosocial needs.
- Develop and implement individualized care plans in collaboration with the interdisciplinary team.
- Provide skilled nursing care, including symptom and medication management.
- Coordinate resources, referrals, and care services for patients and families.
- Offer education and emotional support to patients and their loved ones regarding end-of-life care and planning.
Qualifications:
- Current licensure as a Registered Nurse (RN) in New Mexico.
- Experience in hospice or end-of-life care settings preferred.
- Strong critical thinking and assessment skills.
- Compassionate and empathetic approach to patient care.
- Excellent communication and teamwork skills.
