Job Title: RN Case Manager
Job Type: Full-time, salaried
Job Overview:
As RN Case Manager of Luminos Hospice you will work with the Clinical Director to lead a care team including LPN(s), LVN(s), Aides, MSW and Chaplain to provide best in class hospice care.
Responsibilities:
- Coordinates and oversees all aspects of patients' home care.
- Collaborates with patient care teams to develop and implement care plans.
- Educates families on patient condition, needs and care.
- Performs head to toe physical assessments.
- Checks vital signs and manages medications.
- Administers and maintains catheters.
- Complete documentation timely and accurately.
- Supervises HHAs.
- Regularly communicates patient progress to the clinical manager, physician and care team.
- Other duties as assigned.
- Minimum two years RN experience, hospice preferred
- Valid RN license in the state of Oklahoma
- Current driver’s license, reliable transportation, and valid auto insurance
- Current CPR and BCLS certification
- Excellent verbal and written communication skills
