Job Title: Hospice Aide (CNA)
Job Overview:
A Hospice Aide, integral to the compassionate delivery of end-of-life care, provides essential support to patients and their families in a hospice setting. Duties encompass assisting with activities of daily living, including bathing, grooming, and mobility support, while ensuring patients' comfort and dignity. The Hospice Aide monitors vital signs, communicates effectively with the interdisciplinary care team, and maintains accurate documentation of care activities. Beyond physical care, the role involves offering emotional and spiritual support, fostering a comforting environment during the end-of-life process. Adhering to infection control protocols and participating in continuous education, the Hospice Aide plays a pivotal role in enhancing the quality of life for patients and contributing to our holistic, patient-centered approach to hospice care.
Responsibilities:
- Personal Care Assistance:
- Assisting with activities of daily living (ADLs) such as bathing, grooming, dressing, and oral hygiene.
- Providing assistance with toileting and incontinence care.
- Mobility Support:
- Helping patients with mobility, including transferring in and out of beds or chairs.
- Assisting with ambulation and exercises prescribed by the hospice care team.
- Vital Sign Monitoring:
- Monitoring and reporting vital signs to the nursing or hospice care team.
- Observing and reporting any changes in the patient's physical or emotional condition.
- Comfort Care:
- Ensuring patient comfort through proper positioning and providing pain management assistance.
- Assisting with symptom management under the guidance of the hospice care team.
- Emotional and Spiritual Support:
- Offering emotional support and companionship to patients and their families.
- Assisting with spiritual care based on the patient's beliefs and preferences.
- Record Keeping:
- Maintaining accurate and timely documentation of care provided.
- Reporting observations and care activities to the hospice care team.
- Communication:
- Collaborating with the hospice interdisciplinary team, including nurses, social workers, and chaplains.
- Communicating effectively with patients, families, and other healthcare professionals.
- End-of-Life Care:
- Providing care and support during the end-of-life process with sensitivity and compassion.
- Assisting with post-mortem care as needed.
- Infection Control:
- Following proper infection control procedures to ensure the safety of patients and caregivers.
- Adhering to hygiene and cleanliness standards.
- Continuous Education:
- Participating in ongoing training and education to enhance skills and stay updated on hospice care practices.
- Maintaining certification and licensure requirements.
Hospice Aide (CNA) Requirements
- Strong commitment to improving quality of life while partnering with our community organizations in patient care
- Ability to work well within a team environment to collaborate daily with nurses, social workers, chaplains, volunteers, and Luminos leadership
- Ability to work independently on a day-to-day basis
- Reliable transportation, valid driver’s license, and auto insurance
- State-approved CNA certification
- CPR certified
- Minimum one year of experience as a home health aide; hospice experience preferred.
