Role Description The Oncology Home Infusion Nurse is responsible for delivering high-quality oncology infusion therapies to patients in their homes while ensuring safety, comfort, and adherence to clinical protocols. This role is based in Portland, ME, with a combination of in-person patient visits and some work-from-home for care coordination, documentation, and interdisciplinary communication. Day-to-day responsibilities include conducting patient assessments, administering prescribed infusions, monitoring treatment responses, managing symptoms and side effects, and educating patients and families on therapy plans and self-care. The nurse collaborates closely with oncologists, pharmacists, and other care team members, maintains accurate electronic health records, and follows evidence-based guidelines and infection control standards. The role also involves participating in care planning, contributing to quality improvement initiatives, and supporting the continuous enhancement of Luminate’s home-based oncology services
Qualifications
Graduate of an accredited school of nursing, BSN preferred.
Current state license as a Registered Nurse in good standing.
ONS Provider Card required, OCN desirable.
Minimum of 2 years current nursing experience in a clinical specialty involving infusion therapy, including patient assessment skills, venous access skill and emergency patient care.
Valid driver's license and reliable transportation for home visits
A minimum of 1-year previous specialized home health experience desired.
CPR certification required.
Excellent interpersonal, communication and organization skills.
