Case Manager (MSW) / Service Facilitator

Luma Via Mental Health – Madison, WI (Dane County)

Luma Via is a mental health clinic in Madison providing CCS case management in Dane County. We built this position the way we wish every clinical job was built: competitive pay, real flexibility, and actual support while you get established.

Please Note: Follow the application instructions at the bottom of this post carefully. We use the completeness of your initial application to assign priority to our upcoming interview rounds.

The Details:

  • $55/hr
  • The Target: Target productivity is 30 billable hours per week, with an expected weekly revenue of approximately $1,650. Clinicians have the flexibility to increase their schedule beyond 30 hours, up to a maximum of 40 hours per week if desired. Clinicians who elect to maintain a reduced schedule of 25 billable hours per week may do so; however, eligibility for the monthly health insurance stipend and annual training stipend requires a minimum of 30 billable hours per week.
  • The Ramp-Up Support: We know it takes time to build a caseload through new intakes. To ensure cash flow isn’t a barrier while you grow your billable hours, we provide $4,000 in wage advances during your first 60 days ($2,000 at 30 days, $2,000 at 60 days). This is in addition to the hours you bill.
  • $450/Month "Your Choice" Tax-Free Health Stipend: We skipped restrictive corporate plans. Through an ICHRA, we give you $450/month tax-free to buy the plan you want. Keep your doctors, choose your deductible, and we’ll personally help you set it up.
  • Training Stipend: Eligible after one year of employment, with an average 30 hour per week commitment.

How the build works:

This is a growth-based position. You won't have 30 hours of work on Day 1; instead, your schedule scales up as you complete new intakes (1-2 a month), and build relationships with your clients. Because this takes time, the wage advances are designed to bridge the gap until you hit your full billable target.

Flexible Daytime Hours:

You set your schedule anytime between 7am and 7pm—whatever works for you and your clients. No rigid schedule, no required office hours. Community, home, office, or telehealth—you decide how you work as long as you can meet your clients and work toward that 30-billable-hour goal.

What You’ll Be Doing:

As a CCS Case Manager/Service Facilitator, you’ll work with adults and/or children enrolled in Dane County’s Comprehensive Community Services program—a voluntary, community-based psychosocial rehabilitation program.

Day to Day:

  • Comprehensive Assessments: Completing assessments every 6 months and building individualized recovery plans collaboratively with clients.
  • Consistent Support: Serving as a sounding board and the person who understands the client's "whole picture."
  • Advocacy: Walking alongside clients at medical appointments or housing meetings where the system feels overwhelming.
  • Resource Hub: Connecting clients to providers and community resources.
  • Clinical Variety: Depending on your licensure and interests, you may also provide psychoeducation, skills development, or psychotherapy.

A Caseload That Won’t Break You:

Your target caseload will be 10 adults, 7 children, or a mix of both. This is intentionally smaller than most agencies. We’d rather you do this well than burn out doing it fast.

Beyond core case management, you can pick up additional billable hours through services like Individual Skill Development, Family or Individual Psychoeducation, Group Facilitation, and Employment-Related Skill Training (ERST). These are excellent ways to increase your weekly hours while helping clients with real-life skills like communication, financial management, and finding meaningful work.

What We’re Looking For:

  • Education: Master’s degree in Social Work (MSW) or equivalent required.
  • Experience: Background in case management, counseling, or advocacy (mental health/substance use experience preferred).
  • Skills: Proficiency in assessments, care planning, and active listening. Attention to detail is very important at Luma Via.
  • Approach: Empathy, cultural competence, and a commitment to equity.
  • Logistics: Reliable transportation and Driver’s License preferred.

The Luma Via Pledge:

At Luma Via, we have a simple rule: Leave the pedestal at the door. We refuse to reduce human beings to symptoms or case files. We pledge to treat every individual with genuine humility, recognizing that we have as much to learn from our clients as they do from us. We commit to a culture of "honest awareness", doing the hard work of looking at our own triggers and biases so we can show up fully for the person in front of us. We aren’t here to be the experts; we’re here to be the support. Not the labels. You.
