Mental Health Case Manager / CCS Service Facilitator (Bachelor’s Level)
Salary & Benefits Summary
- Industry-Leading Pay & Growth: Start at $35.00/billable hour with a clear, structured path up to $45.00/billable hour as you grow with our agency. You also get a guaranteed $15.00/hr for administrative tasks, required meetings, and training. Drive time and documentation time for billable hours are included.
- $2,000 Sign-On Repayable Advance (Bonus): Received quickly ($1,000 after 30 days, $1,000 after 60 days) to provide financial peace of mind while your caseload is gradually built with appropriate CCS referrals.
- Job Type: Part-Time (approx. 25 hours/week) with full-time growth potential for those who wish to go over that.

Your weeks will feature a flexible, dynamic mix of telehealth, community-based care, and office-based work tailored to client preferences and your schedule.

Caseload & Hours Expectations
We prioritize client excellence over high-volume burnout. Because of this, our caseload development is highly intentional:
- Gradual Onboarding: Caseloads are built gradually as appropriate intake referrals become available.
- The 25-Hour Benchmark: Once established, team members are expected to maintain a caseload that supports approximately 25 working hours per week.
- Fast-Track Your Financial Growth: Looking for full-time potential or a faster raise? Team members who choose to work more than 25 hours weekly will move through our structured compensation advancement pathway significantly faster, accelerating their path toward the $45.00/billable hour max rate.
- Manageable Numbers: For many Service Facilitators, this will be approximately 10–12 clients, depending on client needs, acuity, authorized hours, frequency of contact, and available referrals.
- Opportunities to Expand: Employees who want additional hours may be able to accept more Service Facilitation clients or provide other CCS services (such as Skill Development or Psychoeducation) based on qualifications, interests, and organizational needs.
The Role: CCS Service Facilitator
As a Bachelor’s-level Service Facilitator within Wisconsin’s Comprehensive Community Services (CCS) program, you will practice strengths-based, recovery-oriented case management. You will serve as the relational anchor and advocate for individuals and families on their path to healing.
Key Responsibilities:
- Relationship-Building & Advocacy: Partner with clients to identify self-directed recovery goals and overcome systemic barriers.
- Care Coordination: Convene and facilitate client treatment teams, collaborating closely with families, county staff, providers, and community partners.
- Resource Connection: Bridge the gap between clients and vital community resources.
- Documentation: Maintain high-quality recovery plans, service authorizations, and progress notes (with extensive guidance and support from your supervisor).
Who You Are (Qualifications):
- Education: Must hold a Bachelor’s degree in one of the following specific fields: Social Work, Psychology, Human Services, or a closely related behavioral science/human services field. - Approach: Possess a trauma-informed, culturally responsive, and recovery-oriented lens.
- Skills: Excellent communication, strong organization, solid documentation skills, and comfort utilizing technology (Google Workspace, email, video platforms).
- Tech-Forward Support: In addition to highly responsive supervision with our Clinic Director (who firmly believes there is no such thing as too many questions!), we provide an extensive, 200 page searchable digital handbook, written visual guides, and video trainings. This framework is a dream come true for tech-comfortable professionals who love having instant, independent access to answers—but it is a poor fit for individuals who dislike utilizing digital resources, video trainings, and manuals.
- Logistics: Possess a reliable vehicle and valid driver’s license for community-based service delivery components.
