Service Coordinator

As a Service Coordinator and a valuable team member, there is an emphasis in listening to the needs of youth and families involved in care. Service Coordinators work collaboratively with members of the team and offer input regarding services through the Lead that are appropriate and available to meet the needs of the family.  Once a collective decision has been reached on the desired service(s), the Service Coordinator will monitor the service delivery ensuring quality of care.   As an integral part of the team, the Service Coordinator is also responsible for documenting the client’s progress and challenges as it relates to goals on their service plan.

Responsibilities Include

• Coordinate and facilitates treatment planning teams, internal collaboration meetings, urgent family team meetings and residential treatment reviews.
• Monitor service implementation in individual cases
• Ensure coordination among network providers in individual cases
• Facilitate utilization review process for support and stabilization services
• Monitors timely posting of treatment plans and critical incidents.
• Provide written summaries of treatment team meetings in the Virtual Gateway
• Ensures communication among DCF staff, provider agencies and families as needed
• Respond to crisis management needs of the DCF Area Office
• Establishes working relationships with foster families, program staff and other individuals or groups to meet the specific needs of the child and family
• Assists in the appropriate discharge process and after care as formulated in the service plan
• Process referral packets as needed
• Seek emergency placements as needed
• Conduct CANS assessments
• Lead discharge planning process with providers, DCF and families.
• Provide data or reports to Lead Director as needed.

Knowledge and Experience Required

• Bachelor’s Degree in Human Services or related field
• Experience working with youth and families
• Case Management Experience
