Role Description
This is a full-time, on-site Social Worker role based in Lubbock, TX. The Social Worker will play an integral part in supporting the emotional, social, and mental well-being of patients, providing counseling, connecting individuals to community resources, and coordinating care plans. The candidate will work closely with healthcare professionals to assess patients’ needs and develop strategies to support their overall health and wellness.
Qualifications
- Possess strong interpersonal, communication, and counseling skills.

- Proficiency in conducting needs assessments and developing care plans tailored to individuals and families.
