Job Description
Behavioral health service provider s is seeking compassionate and motivated fully licensed clinicians (LCSW, LPC, LMFT) to provide community-based behavioral health services in the Macon/Warner Robins area . Services are delivered in clients’ homes, schools, and other community settings to individuals diagnosed with mental health, substance use, and/or co-occurring disorders.
This role is ideal for clinicians who are passionate about making a direct impact in the community and are comfortable working both independently and collaboratively.
Responsibilities include (but are not limited to):
- Conduct Biopsychosocial Assessments, Behavioral Health Assessments, Re-Assessments, and Diagnostic evaluations
- Develop Individual Recovery Plans (IRPs)/Treatment Plans based on the assessed needs and strengths of the individual and family
- Provide clinically appropriate, person-centered services in community settings
- Maintain timely, accurate, and compliant clinical documentation in accordance with agency, state, and federal guidelines
- Collaborate with interdisciplinary team members and community partners to ensure continuity of care
- Manage caseload effectively to meet client needs and productivity expectations
- Participate in staff meetings, trainings, and supervision as required
Qualifications:
- Active, unrestricted Georgia license (LCSW, LPC, or LMFT)
- Experience working with individuals with mental health, substance use, or co-occurring disorders preferred
- Ability to work independently in community-based settings
- Strong clinical, organizational, and documentation skills
- Reliable transportation and willingness to travel within the community
